- Severity:
- “On a scale from 1 to 10, how would you rate the pain?”
- Character:
- “Can you describe the pain? Is it burning, throbbing, stabbing, constricting, tight, colicky, or something else?”
- Relieving Factors:
- “Is there anything that seems to ease the pain?”
- Exacerbating Factors:
- “What activities or conditions make the pain worse?”
- Possible Cause:
- “Do you have any idea what might have caused this pain? Did you have any injury or trauma?”
Swelling Analysis:
- Site:
- “Where exactly is the swelling located?”
- Onset:
- “Did the swelling appear suddenly or develop gradually?”
- Course:
- “Have you noticed any change in the size of the swelling over time?”
- Duration:
- “When did you first notice the swelling?”
- Other Swellings:
- “Do you have any other swellings on your body?”
- Relation to Pain:
- “Is the swelling associated with any pain?”
- Possible Cause:
- “Do you have any thoughts on what might have caused the swelling?”
Ulcer Analysis:
- Site:
- “Where is the ulcer located?”
- Onset:
- “Did the ulcer appear suddenly or develop over time?”
- Course:
- “Has the ulcer changed in size since it first appeared?”
- Duration:
- “When did you first notice the ulcer?”
- Relation to Other Symptoms:
- “Is the ulcer associated with any other symptoms like pain, swelling, or both?”
- Possible Cause:
- “Any idea what might have caused the ulcer?”
- Constitutional Symptoms:
